Dear Judy:
The Minnesota Department of Health (MDH) has requested that SEH field locate and GPS coordinates of
private wells and collect water levels within the City of St. Joseph Wellhead Protection Plan model
domain as data calibration points for the MLAEM model. SEH has not previously been required by the
MDH to perform this task; therefore, the proposed new scope of work was not included in the original
proposal dated December 18, 2003.
In consideration of MDH's request, SEH is requesting payment for additional services in the amount of
X3,050.00 for the additional work requested by the MDH in order to complete Phase 1 of the Wellhead
Protection Plan for the City of St. Joseph. This cost includes travel to St. Joseph, location of wells,
downloading of data, verification of point locations, and the inclusion of the points and water levels into
the existing groundwater model as outlined on the enclosed spreadsheet.

forward to finishing the WHPP in a timely manner.
