Kay's Kitchen, located at 303 College Avenue North, St. Renee Symanietz Joseph, is seeking a BFA grant to assist with the purchase and installation of am 8' x 16' mural Dale wiok to be installed on the north facing wall of Kay's Kitchen.. Illustrations are included in the packet. EDA Larry Hosch The total project cost is estimated at$2,950. A copy of the quote is enclosed. A$1,000 grant is Doug Danielson requested. Steve Frank Tom skal,en Upon reviewing the application as it relates to the BFA Eligibility Requirements it is noted: Dale wi�k • The proposed project is an eligible item as it is visible from a public right-of-way. It appears consistent with the intended goal to further revitalization/renewal efforts. • The property is current with taxes and assessments. • The property is zoned commercial and is consistent with the City's Zoning Ordinance and Comprehensive Plan. • The applicant has not received a BFA grant in 2012 (BFA policies limit properties to one grant per calendar year). • A sign permit will be required as a condition, in accordance with the policies. Ms. Finken will be in attendance at the EDA meeting to respond to any questions the EDA may have The EDA's 2012 capital allocation included funding for BFA Grant applications and other revitalization efforts. The August 31, 2012 fund balance was $7,160. BFA program oversight and authority for grant approval was delegated to the EDA. Action: EDA consideration of the grant appfication is kindly requested. It is recommended the EDA approve the grant contingent upon obtaining the required sign/ building permits, with funds dispersed to the applicant upon submittal of receipts or invoices for supplies purchased and inspection which certifies the work completed is in accordance with the EDA approval and other city ordinances. In the case of improvements that do not require a city- issued building permit, the applicant must submit the name and state license number of the contractor completing the work and agree to an inspection of the work by a city building official. Grant Disbursement Awarded loan funds wiil be dispersed to the applicant upon submittal of receipts or invoices for supplies purchased and inspection which certifies the work completed is in accordance with the EDA approval and other city ordinances. Certification City staff or an authorized representative shall have the right to inspect the property to be improved at any time from the date of application upon giving due notice to the owner and to occupants. I/We understand that any intentional misstatements will be grounds for disqualification. I/We authorize program representatives the right to access the property to be improved for the purpose of the grant program and to take photographs of the structure before and after rehabilitation. I/We further understand that I/we will make the final selection of the improvements to be made with the loan funds and that the contract for improvements will be solely between myself and the contractor(s). The administering agency will not be liable for the inadequate performance of the contractor(s). The information on this application is accurate.
